The Primary care routine pathway within the Comprehensive Emotional Care Mental Health Team has an exciting opportunity for an experienced adult mental health nurse to join our team. Do you have a passion for working within adult mental health? Then come and join the team and meet likeminded people. This role will involve offering triage, assessments, reviews and interventions for patients referred to the team, in accessible locations across North Lincolnshire. Networking between Primary Care and Secondary Care Services is a central part of the role to ensure the patients journey between services is seamless and care and treatment is provided in a timely manner. The role will also involve working with the wider team to triage and offer routine mental health assessments to adults experiencing mental health problems within Primary Care. Main duties of the job: As a member of the Team the post holder will provide high quality nursing assessment and interventions within the primary care routine Pathway. The post holder will be an enthusiastic, highly organised and motivated mental health professional who can provide high quality nursing expertise and support to patients. It is essential that the applicant can demonstrate post registration experience as an RMN, which may include inpatient and community settings.
