Overview

We're looking for Community Mental Health Nurses (Care Co‑coordinator / Key Worker) to join a dedicated team providing specialist mental health care to the population of South Tameside.

The Secondary Care CMHT supports residents with severe and enduring mental health conditions. We aim to provide outstanding care and continuously improve in response to the needs of the population we serve.

Transformation

This is an exciting time for the CMHT. We are undergoing a transformation as part of national community service developments and local Living Well initiatives. It's an opportunity to participate in service development with planned investment in staff and new role development.

Role Scope

The CMHT works with people aged 16 and over with severe and enduring mental health and social care needs, collaborating closely with the local authority and a multidisciplinary team.

Key Responsibilities
  • Provide holistic assessment, interventions, risk assessments and care plans for people on your caseload.
  • Assessment of health care needs.
  • Produce collaborative care plans.
  • Risk assessment and management.
  • Monitor and evaluate care plans.
  • Liaise with wider MDT.
  • Provide evidence‑based therapeutic interventions.
  • Offer education, advice, emotional and psychological support.
  • Promote choice in service user involvement in care planning and risk management.
  • Ensure practice aligns with social inclusion and recovery models.
  • Ensure carer involvement in the service user’s care.
  • Report concerns identified under Safeguarding adult procedures.
  • Maintain accurate records.
