Overview

This is a Band 6 Community Learning Disability Nurse position within Wrexham. The post holder will work as part of the Learning Disability team based at Wrexham, ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in patient services, as well as other statutory agencies and the voluntary sector. The post holder will deliver evidence based, safe effective interventions and clinical care, following the nursing process. The post holder will follow the service principles and strategy for people with a learning disability while delivering safe, effective and high-quality specialist service in line with agreed standards. The post holder will have excellent communication skills and will be a visible and accessible member of the team, patients and their families can access for assistance, advice and support. The post holder will be expected to communicate and receive highly complex and sensitive information and able to analyse and interpret information and act appropriately where there may be barriers to acceptance. The post holder will have a good understanding of the CHC process and also be part of the development of pathways and Maturity Matrix. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
