Job Overview

Hampshire and Isle of Wight Healthcare NHS Foundation Trust -- we have an exciting opportunity for you! We're looking for dedicated individuals to join our thriving Community Nursing Service in Portsmouth, Fareham and Southampton in a permanent role, delivering outstanding care to the community in a supportive and collaborative environment. Our Community Nursing Service provides high-quality nursing care to patients in their own homes, operating 12 hours a day (8am--8pm), 7 days a week, 365 days a year. We pride ourselves on delivering person-centred care and ensuring our staff feel valued, supported, and encouraged to develop their skills. In this role, you'll work as part of a wider multi-disciplinary team, contributing to care planning, evaluation, and effective communication across services.

Responsibilities
  • Work under supervision following written care plans and care pathways devised by the Registered Nurse.
  • Follow prescribed care plans and treatment protocols.
  • Follow HPCT Policies and Procedures.
  • Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.
  • Delegated responsibility for Patient Support Teams administrative tasks.
  • Provide general administration support to the team.
  • A full driving license and access to a vehicle is required for this role.
