Job summary

Job description
Overview

We are expanding our frailty hospital at home (FH@H) and older persons service and are looking for a Community Healthcare Assistant to join our team. In this role you will provide support to adult patients in their own homes, promote independence, improve quality of life and assist patients to remain at home while supported by a multi‑disciplinary team of nurses and therapists.

Responsibilities
  • Assist patients to regain independence and achieve set goals.
  • Provide administrative support to ensure efficient day‑to‑day running of the service.
  • Provide rehab‑based care, including a degree of personal care in the short term.
  • Monitor deteriorating patients with support from clinicians.
  • Support patients to remain at home and communicate effectively with the multi‑disciplinary team.
Qualifications & Requirements
  • Valid right to work in the UK; visa sponsorship is not available.
  • Enhanced Disclosure and Barring Service (DBS) check required (the Trust will cover the cost).
  • Applicants with an existing DBS update service can present a valid DBS certificate.
  • A valid driver’s licence and the ability to drive are essential.
  • Strong communication skills and confident decision‑making.
Training

Training will be provided to reach competence in clinical observations, venepuncture, bladder scanning and ECG as required.

Working Hours

Full‑time or part‑time positions are available. Current hours are 08:30‑16:30, five days a week, with a view to extending to a 7‑day service of 08:00‑20:00.
