Community Health Care Assistant - Chertsey/Weybridge

Permanent position, working 37.5 hours per week, 9:00AM to 5PM, may be subject to change, working Monday to Friday.

The post holder will be required to work at St Peter's Hospital, Guildford Road, Chertsey, Surrey, KT16 0PZ, and at Weybridge Health Centre, 22 Church Street, Weybridge, Surrey, KT13 8DW

Salary: Agenda for Change Band 5 Equivalent

The post holder will work as part of our team to provide high-quality care to the patients of our 37 practices across our Federation. The Health Care Assistant will work autonomously to deliver safe and effective nursing care. Previous primary care experience is essential.

Job responsibilities

Job Summary

The post holder will work as part of our team to provide high-quality care to the patients of our 37 practices across our Federation. The Health Care Assistant will work autonomously to deliver safe and effective nursing care. Previous primary care experience is essential.

Key Responsibilities

  • Wound care and dressings
  • NHS Health Checks and other preventive health activity
  • Chronic disease monitoring and reviews within scope of competence

Clinical Duties

  • Undertake venepuncture and ensure high standards of practice across the Federations phlebotomy service.
  • Deliver wound assessment and management, including dressing changes and post-operative care, including removal of clips and sutures.
  • Conduct NHS Health Checks and contribute to proactive health promotion initiatives.
  • Undertake chronic disease reviews (e.g., hypertension, diabetes, COPD, asthma) in line with protocols and within scope of competence.
  • Accurately record clinical data on the relevant clinical systems (EMIS).
  • Escalate concerns or clinical issues appropriately to the supervising Lead Nurse or GP.
  • Recognise deterioration in wounds and refer onto the complex wound care clinic as necessary.

  • Maintain accurate, contemporaneous, and confidential patient records.
  • Adhere to infection prevention and control policies and procedures.

  • Collaborate with NICS colleagues to support integrated patient care.
  • Champion our NICS services with practices and PCNs including attendance at meetings where appropriate and valuable.
Person Specification
Experience
  • Experience of working autonomously.
  • Recent experience working in a clinical environment delivering direct patient care.
  • Experience of phlebotomy and wound care.
  • Experience working as part of a wide team.
  • Experience conducting health checks or chronic disease reviews.
Qualifications
  • Previous Primary care experience.
  • Phlebotomy trained and competent at working autonomously.
  • Wound care experience.
  • Evidence of ongoing professional development and mandatory training compliance.
  • Experience conducting health checks or chronic disease reviews.
Knowledge & Skills
  • Knowledge of infection prevention and control, safeguarding, and confidentiality.
  • Ability to carry out phlebotomy, wound care, and other clinical procedures safely.
  • Competent IT skills, including use of EMIS.
  • Effective communication and interpersonal skills with patients and colleagues.
  • Understanding of Quality and Outcomes Framework (QOF) and primary care pathways.
  • Awareness of long‑term condition management principles.
  • Understanding of audit and quality improvement methods.
