Overview

If you are looking for a paediatric nursing role where you can combine acute nursing skills with community work, read on. An exciting opportunity has arisen to join the friendly and progressive Children\'s Home Acute Team (CHAT). CHAT is an innovative nursing team that sits within an integrated child health team and delivers acute and ambulatory care at home to prevent hospital admission and facilitate early discharge.

We are looking for an experienced paediatric nurse who can work autonomously to care for acutely unwell children at home and be part of a developing community nursing team. You will deliver excellent acute nursing care in the community in and around Hampshire and join a high level of nursing expertise, acting as a role model to the nurses and students within the team. The post holder will have acute and preferably community experience.

This challenging and rewarding role offers an opportunity to develop your acute skills while working in both the acute and community settings. You will work as part of the CHAT to develop ambulatory and unscheduled care pathways, prevent unnecessary admissions, and facilitate early discharge from inpatient wards. Flexibility for working arrangements will be considered.

  • Demonstrate high professional standards and act as a role model for colleagues and students.
Qualifications

  • Ability to work autonomously and as part of a multidisciplinary team.
