An exciting opportunity has arisen for an experienced Nurse to join our innovative and forward‑thinking Integrated Care Team of Community Nurses, Advanced Nurse Practitioners, Healthcare Assistants, Social Care and Allied Health Care Professionals in Perth and Kinross. In this role you will collaborate closely with a range of services and care for a diverse group of patients, helping deliver the right care, in the right place, at the right time. We prioritise flexible, agile working while maintaining high‑quality care, seven days a week. Our vision is to reshape community healthcare in Perth and Kinross, reducing hospital visits, embracing digital solutions, and supporting people to live healthier, more independent lives.

Responsibilities
  • Carry out comprehensive assessment and triage of patient referrals, ensuring an appropriate care pathway is established for everyone.
  • Coordinate escalation of care to improve patient outcomes and reduce the need for unnecessary emergency admission or readmission to hospital or long‑term care.
  • Ensure efficient patient flow between acute care and a wide range of community services to facilitate safe, timely discharge.
  • Prevent deterioration, prevent unnecessary hospital admission and facilitate discharge or transfer to another facility by coordinating multidisciplinary and multiagency care team requirements.
  • Support the wider community nursing team, optimising patient care while maintaining your skills.
  • Work a variety of shifts, including weekends; hours of work are Monday – Sunday, 0800 – 2000 hrs.
Qualifications and Experience
  • Previous experience of working in community nursing.
  • Excellent verbal and non‑verbal communication skills and the ability to motivate service users.
  • Energetic and flexible approach to work, ability to work well within a team and work without direct supervision.
  • Wide range of clinical knowledge and skills and evidence of continuous professional development.
  • Ability to work well within a team is essential along with the ability to work without direct supervision.
  • Leadership qualities and excellent communication skills.
  • Must be caring, professional, work well in a high‑pressure environment and easily adapt to changing situations.
