Overview

Community Case Manager RNLD - Supported Living, Bristol. We are seeking an experienced Community Case Manager RNLD to join a respected provider of complex care for adults and children with learning disabilities and complex needs across Bristol and the surrounding areas. This is a full-time, permanent role, Monday to Friday (9-5), with hybrid working available — two days from home and three days in the community. The position offers a competitive salary of £50,000-£55,000 DOE.

Responsibilities
  • Manage and oversee supported living care packages for individuals with learning disabilities and complex needs.
  • Conduct holistic assessments and develop person-centred care plans tailored to each individual\'s needs, preferences, and goals.
  • Support and mentor nurses and support staff to deliver outstanding care and promote independence for service users.
  • Act as the main point of contact for families, commissioners, CHC teams, and local authorities.
  • Lead on clinical governance, safeguarding, and quality improvement initiatives within supported living settings.
  • Ensure regulatory compliance and that all care is delivered in line with CQC standards.
What We\'re Looking For
  • Registered Learning Disability Nurse (RNLD) with active NMC registration.
  • At least 2 years\' post-qualification experience, ideally within supported living or learning disability services.
  • Strong understanding of positive behaviour support and person-centred care planning.
  • Full UK driving licence and access to a vehicle; willing to travel across Bristol and surrounding areas.
