Social Care Practitioner for Occupational Therapy. Working in the High Peak and Derbyshire Dales areas. Face-to-face visits in the community are an integral element of this role. The purpose is to support a backlog of non-complex Occupational Therapy waiting list assessments. Candidates do not need to be a qualified Occupational Therapist but should have a Level 3 Diploma in Adult Care.
The post holder will have strong assessment skills, work closely with the person’s assets, an equipment provider, and Housing and Health departments to help people remain independent at home through small aids and minor and major adaptations. The role requires a proven track record of working in a Local Authority environment and experience of Occupational Therapy interventions, including equipment solutions and housing adaptations, in partnership with housing providers.
You will be supported by an experienced management team, a wider network of Social Care Practitioners, Occupational Therapists, Team Coordinators, and a dedicated Occupational Therapy service manager and senior practitioners. A strength-based approach is expected to deliver a proportionate, personalised, safe and sustainable response for local adults, their carers and families. A positive approach to risk taking, accurate record keeping, personal resilience, and the ability to contribute to inter-agency and multi-disciplinary work are key.
Working days: Full time.
Work with clients and their families/carers to identify and mobilise strengths, networks, and communities to maximise independence, self-care, social and occupational opportunities as appropriate. Work with other professionals including social workers and occupational therapists, referring on as necessary. Assess and review individual needs and arrange or refer services; including prescribing and delivering simple equipment and non-complex housing adaptations as needed. Review work and caseload with senior practitioner/service manager at regular intervals, including case closures or transfers. Undertake holistic assessments for individuals with less complex needs to identify eligible outcomes and support viable care and support plans in line with legislation. Share information with clients about resources and link people to local community resources/activities. Liaise with voluntary bodies, residential and day establishments, and other agencies. Ensure safeguarding of vulnerable adults.
