Job Summary

We are looking for passionate and skilled Band 6 Community Nurses to join our Thanet Community Nursing Teams, based at our brand new £10million state‑of‑the‑art Thanet Health Hub.

Our award‑winning service brings together a skilled group of nurses, physiotherapists, exercise physiologists, counsellors, cardiac rehab assistants, practitioners, an occupational therapist, administrators, and volunteers.

In this role you will deliver cardiac rehab services across an allocated area of East Kent and help cover other areas based on demand, waiting times, and capacity.

You will also work closely with the Senior Clinical Lead and Head of Service to ensure safe, high‑quality care through an effective and efficient model.

Responsibilities
  • Assess, deliver, and evaluate care for patients with complex, long‑term, and potentially high‑risk cardiac conditions.
  • Manage a caseload of patients with ongoing needs who would benefit from targeted case management.
  • Serve as a go‑to resource on Coronary Heart Disease for primary, community, and secondary care services.
  • Support flexible working arrangements to meet patients and staff needs.
Qualifications
  • Registered Health Care Professional (NMC registration)
  • Level 6 degree or equivalent qualification, or equivalent knowledge gained through experience plus a diploma in nursing
  • A full, valid UK driving licence (or valid driving licence issued by an EU country)
  • Recent cardiology experience
  • Use of IT systems and databases
Desirable (Optional)
  • Knowledge of up to date clinical practice, coronary heart disease and secondary prevention including medication optimisation
  • Awareness of NHS developments, National Service Frameworks and NICE guidance
