A healthcare provider in North Lincolnshire is looking for a dedicated individual to deliver high-quality care for patients recently discharged from hospital. This role requires assisting with personal care, administering medication, and supporting patient independence. Ideally, candidates need an NVQ qualification and the ability to work four visits a day across seven days. Flexible working arrangements are available to support work-life balance, and an essential requirement includes maintaining dignity and respect in patient care.
Come and join an exciting service within North Lincolnshire who support patients when leaving hospital that require short term care and rehabilitation. The service will provide care support and re-enablement for patients and their families across seven days a week.
The team will work in collaboration with the District Nurses, locality coordinators, discharge teams and will closely link with community therapy services to promote functional independence and rehabilitation for short term care and support needs.
