Overview

Are you looking for a new challenge? An exciting opportunity has arisen for an enthusiastic and motivated individual to join the Urgent Neighbourhood Services, Urgent Community Response Team on a temporary basis as a Clinical Team Lead. The Urgent Neighbourhood Services Directorate sits within Urgent Community Response (UCR), Virtual Ward (VW), Neighbourhood ACPs and Stockport Digital Hub. As a Clinical Team Lead you will primarily work within UCR, VW and Neighbourhood ACPs. UCR is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home who are experiencing acute illness or sudden change in function. The VW can clinically monitor patients remotely using enhanced technology which is supported by the Digital Hub team.

The post holder will be responsible for leadership and management of the team to ensure excellence in clinical care for patients accessing Virtual Ward, Urgent Community Response and Advanced Clinical Practitioner services.

Responsibilities
  • Provide effective leadership, monitor professional and care standards, and ensure effective use of resources within the Directorate.
  • Support and develop health and social care integration across sharing responsibilities for leadership and management of staff in multi-professional teams.
  • Be a visible and accessible presence in the clinical area where appropriate.
  • Act as role model and mentor.
  • Understand the clinical pathways aligned to Urgent Community Response and Virtual Ward, and ensure the deteriorating patient is appropriately escalated.
  • Ensure clinical competency frameworks are in operation.
  • Be responsible for providing compassionate, safe and high-quality patient care.
  • Actively contribute to and support agreed developments within the Directorate with a focus on clinical effectiveness and innovative practice.
  • Manage roster allocations for present and future shifts to provide safe staffing levels in line with the agreed workforce model.
  • Lead the professional team daily, using resources effectively and efficiently.
  • Escalate concerns about patient safety, quality and experience in a timely manner, offering solutions and evidence-based recommendations.
  • Collaborate with the organisation to reflect the community served and participate in opportunities to improve diversity within the workforce.
