An exciting opportunity has arisen for an enthusiastic and motivated individual to join the Urgent Neighbourhood Services, Urgent Community Response Team on a temporary basis as a Clinical Team Lead. The Urgent Neighbourhood Services Directorate sits within Urgent Community Response (UCR), Virtual Ward (VW), Neighbourhood ACPs and Stockport Digital Hub. As a Clinical Team Lead you will primarily work within UCR, VW and Neighbourhood ACPs. UCR is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home who are experiencing acute illness or sudden change in function. The VW can clinically monitor patients remotely using enhanced technology which is supported by the Digital Hub team.
The post holder will be responsible for leadership and management of the team to ensure excellence in clinical care for patients accessing Virtual Ward, Urgent Community Response and Advanced Clinical Practitioner services.
