A primary care medical practice in London seeks a Clinical Pharmacist Independent Prescriber to join their healthcare team. The ideal candidate will manage chronic disease management and conduct clinical medication reviews while liaising closely with GPs and nurses. This role requires registered status with the General Pharmaceutical Council and experience in primary care. A supportive environment is ensured for professionals looking to develop their clinical skills. Salary range is £52,000 to £60,000 annually, with a pro-rata option for part-time hours.
Portland Medical Centre, as part of Bourne Health are looking for a proactive and enthusiastic Clinical Pharmacist who is already an established Independent Prescriber, to join our Practice to work onsite closely with our nurses and GPs.
The appointed Pharmacist would be part of a wider pharmacy team working with our associate practices within a Hub model.
As the role will develop the successful candidate will take responsibility for areas of Chronic Disease Management and undertake Clinical Medication Reviews, proactively manage patients with complex poly-pharmacy needs. The role would also entail providing primary support to GP staff with regards to prescriptions (this includes repeat prescribing), medication queries, complex clinical queries – please see full job description. We will look to provide support to develop and train the candidate to further their skills in their clinical interests. The Pharmacist would have an allocated GP Partner to mentor, support and guide them.
