Qualifications

  • Completion of an undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council.
  • Experience conducting structured medication reviews with polypharmacy patients.
  • Experience in a General Practice setting.

Responsibilities

  • Conduct structured medication reviews and support patients with long-term conditions.
  • Manage acute and repeat medication requests including medication reconciliation.
  • Lead clinical audits and drive improvements in prescribing practices.
