Join our forward-thinking team as a Clinical Pharmacist working directly in one of our Mid Norfolk PCN Practices - Shipdham Surgery.

Our Practices are committed to delivering exceptional patient care through innovation and collaboration. Our multidisciplinary team values the unique contributions of clinical pharmacists in enhancing medication management and patient outcomes.

The candidate will report into and receive clinical supervision from a GP/Partner.

Main duties of the job

As a Clinical Pharmacist within our Practices, you will:

  • Conduct structured medication reviews (SMRs) for patients with complex needs.
  • Manage long-term conditions such as hypertension, asthma, and diabetes.
  • Provide expert advice on medicines optimization and safety.
  • Support repeat prescription processes and medication reconciliation.
  • Collaborate with GPs, nurses, and other healthcare professionals to deliver integrated care.

Who We're Looking For

  • Registered with the General Pharmaceutical Council (GPhC).
  • Primary Care Pharmacy Education Patchway (PCPEP) qualification - Essential
  • Independent Prescriber Qualified - Desirable
  • Experience in primary care setting
  • Strong communication and interpersonal skills.
  • Commitment to continuous professional development.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to improve the quality and safety of care offered to patients of Shipdham Surgery.

Details of hours.

19 hours per week to include : Wednesday 1000-2000 and Friday 0800-1800.

Experience
  • Minimum of 2 years post qualifying experience as a pharmacist.
Qualifications
  • Completion of an undergraduate degree in pharmacy.
  • Working towards an independent prescribing qualification
  • Primary Care Pharmacy Education Patchway (PCPEP) qualification
  • Demonstrates understanding of and conforms to, relevant standards of practice
  • Follows professional and organisational policies/procedures relating to performance management
  • Holding an independent prescribing qualification
  • Clinical Diploma
