Job description

We are looking for an experienced, enthusiastic prescribing pharmacist. We are a friendly and supportive primary care team comprising GPs, ANPs, nurses, physios and pharmacists. We have multicultural patient population of around 8000 patients. We are a Deepend practice, which means we are situated in one of the most socioeconomically deprived areas of the country. Our patients struggle more than most; the work that we do can have a huge impact on their health and wellbeing. We have regular team meetings and daily multidisciplinary catchups. Everyone plays their part equally to address Health Inequalities, if you want your work to make a difference, we are the place for you!

Main duties of the job

You will work with patients, either in person or via telephone consultation to carry out structured Medication reviews, deal with medication queries, titrate medication and initiate medication for conditions such as hypertension and lipid management. You will carry out more complex medication reconciliations and deal with prescription queries. You will be responsible for Medicines optimisation and quality improvement. You will have an awareness of our prescribing budget and how to adhere to this. You will carry out audits as necessary to assist the practice to achieve QOF, QIPP and Locally commissioned service targets.

You will work alongside another prescribing pharmacist and a pharmacy technician. You will work with our network pharmacy team to ensure joined up working and realisation of shared agendas

About us

We are a 'green' practice, where we try to make environmentally friendly decisions. We are a relatively small team - around 20 people, ranging from those who have been with us 30 years to people currently with us covering maternity leave, the average length of service is around 10 years, we don't have a high staff turnover. The management team consists of 4 partners and a practice manager. The atmosphere is warm and welcoming, with all designated roles part of one team. There are regular coffee breaks during the day and a fruit bowl provided for staff ( we do have a biscuit cupboard if you prefer!!). We work hard, and we deal with many complex cases, but staff welfare is a priority, you will find both peers and team leaders very supportive

Job responsibilities

OverallPurpose

To provide prescribing advice and support to the practice(s), on all medication issues.

To advise the practice on the implications of prescribing and admission/referral data provided by the CCG.

To implement change in prescribing behaviour, where required, in order to promote quality, rational, safe and cost-effective prescribing, including deprescribing and complex polypharmacy.

To promote prescribing in line with national, and CCG guidelines and policies.

To work in line with the new GP contract and GP Network with regard to quality improvement and implementation of policies.

To identify and advise on any areas of clinical risk connected with prescribing.

To undertake medication reviews and therapeutic specific medication reviews.

To work directly with patients on medication issues, face to face, telephone, home visits and patients in care homes

To consider specific practice related BAME prescribing issues such as Ramadan and vitamin D

Potential to hold minor ailments clinics

Responsibility for Patient Care (including monitoring, diagnostics + investigations)

To support all practice staff on all medication issues

To update patients medication records post hospital discharges and outpatient appointments, and advise the GP of all issues that may arise

To implement all monitoring requirements as detailed in agreed shared care protocols.

To be responsible for ensuring recall systems are in place, and working and for the correct monitoring of amber drugs and those included in the Shared Care Local Enhanced Service.

To deal with medication queries, especially those that require follow up and problem solving techniques.

To participate/run medication review clinics or chronic disease management clinics as agreed with the practice.

To reduce waste, minimise risk and improve the repeat prescribing process.

To ensure prescription security by introducing best practice guidelines

To resolve all practice EPS issues queries and ensure that the practice maximises the benefits of EPS.

Identify patients who would be suitable for repeat dispensing and pharmacy MURs.

To ensure information from pharmacy MURs is acted upon as appropriate.
