Clinical Pharmacist – Primary Care / GP Practice (Full-Time or Part-Time)

About the Role

We are looking for a motivated Clinical Pharmacist to join our GP Practice/PCN team. This is an excellent opportunity for a pharmacist with experience in primary care, hospital or community pharmacy who wants to develop advanced clinical skills, conduct medication reviews, and support safe, effective prescribing.

This role is ideal for someone who enjoys working autonomously, improving patient care, and being a key part of a multidisciplinary team.

Key Responsibilities
  • Conduct Structured Medication Reviews (SMRs) for patients with chronic conditions, polypharmacy, frailty, or complex needs.

  • Provide medicines optimisation and deliver cost-effective, safe prescribing.

  • Support long‑term condition management (hypertension, asthma, COPD, diabetes).

  • Review and manage high‑risk medications (e.g., anticoagulants, DMARDs).

  • Deliver patient‑facing clinics, medication counselling, inhaler technique, and lifestyle support.

  • Review hospital discharge summaries and clinic letters to ensure safe medicines reconciliation.

  • Provide clinical support and advice to GPs, ANPs, Practice Nurses, HCAs and administrative teams.

  • If independently prescribing: initiate, adjust, and deprescribe medications safely within scope.

  • Contribute to QOF, audits, and quality improvement projects.

What We’re Looking For
  • GPhC registered Pharmacist

  • Experience in primary care, hospital pharmacy or community pharmacy

  • Strong understanding of medicines management and clinical guidelines

  • Excellent communication and consultation skills

  • Ability to prioritise workload and work independently

  • Confident working within a multidisciplinary team

Desirable (Training Can Be Provided)
  • Independent Prescribing (V300)

  • Experience with EMIS, SystmOne or similar clinical systems

  • Experience completing SMRs or monitoring high‑risk medication

  • Training in clinical assessment or chronic disease management
