Job description

The post holder will work with the wider multi‑disciplinary team to manage and co‑coordinate the provision of diabetes nursing care to the renal inpatient and outpatient populaiton.

You will be responsible for leading a specialised service, providing nursing input as a clinical expert to support and facilitate high quality, individualised care.

You will be a resource for patients and the clinical team, acting a professional role model providing direct and indirect care.

  • Provision of specialist, high quality, expert nursing care and support for renal patients (and their carers) with diabetes in liaison with the multidisciplinary team(s) and other health agencies.
  • To act as a resource for patients and the clinical team by the provision of comprehensive advice, information, teaching and training with regards to diabetes in kidney disease both within ICHT and externally.
  • To undertake, where appropriate, nursing research with published outcomes, and updating own knowledge to promote excellence in clinical practice, and the setting of quality standards within speciality.
  • Contribute in audit processes. Formulate an annual report/clinical audit of own CNS service and outcomes.
