About the Role

As the Dementia & Delirium Clinical Nurse Consultant, you’ll provide advanced clinical leadership, expert consultancy and education across Tamworth Hospital for patients with cognitive impairment. You’ll support teams in ED, medical, surgical, rehabilitation, mental health and the older persons’ dementia unit.

Your role includes expert assessment, care planning, coaching staff, implementing evidence-based pathways, supporting prevention and early recognition of delirium, leading quality improvement, and strengthening partnerships with aged care and community services.

This is a highly visible role where your expertise will directly influence safer, more compassionate care for older people across the hospital. You won’t be alone in this role! From day one you will receive a structured orientation, ongoing mentorship, access to statewide dementia/delirium networks and education, professional development, workshops and Clinical Nurse Consultant forums.

Who You Are

You are a clinically strong, person-centred nurse leader with a passion for improving outcomes for people with dementia, delirium and cognitive impairment. You bring:

  • Current AHPRA registration
  • Significant recent experience in gerontology, acute care of the older person, mental health and/or dementia/delirium care
  • Advanced assessment, care planning and consultancy skills
  • Expertise in evidence-based dementia and delirium practice, including BPSD management
  • Strong communication skills with the ability to engage patients, carers, and stakeholders with sensitivity
  • Proven ability to mentor, educate and build capability across the workforce
  • Confidence leading change, translating evidence and data into practice, and driving improvement initiatives
  • A collaborative, approachable style and commitment to culturally safe, person-centred care
