Job description

The Navigator will work autonomously, possess excellent communication skills and have experience and knowledge of working with Crisis/Rapid Response and community services within primary or secondary care services. The post holder will be expected to undertake holistic assessment of patients using a person‑centred approach, develop appropriate care packages across health and social care to avoid inappropriate admission to hospital or long‑term care environments, and facilitate the timely discharge of patients from secondary care.

Key Responsibilities
  • Work autonomously and communicate effectively with multidisciplinary teams.
  • Undertake holistic assessments of patients, including mobility and functional assessments.
  • Diagnose and formulate individual treatment plans and programmes for patients referred to the navigator service.
  • Identify, sign‑post, assess and coordinate patients deemed appropriate to alternative community and outpatient services from the A&E department and Medical Assessment Unit.
  • Liaise closely with clinicians and professionals working in emergency care.
  • Incorporate specialist skills alongside nursing and clinical staff within emergency departments.
  • Analyse clinical and non‑clinical findings to form accurate diagnoses and prognoses in a wide range of complex conditions, including the use of standard tests where appropriate.
  • Identify and manage risk with regards to function, mobility and cognition.
  • Recommend and implement interventions, providing or supporting comprehensive case management, treatment and discharge plans accordingly.
Qualifications and Experience

The post requires experience and knowledge from a health‑related professional or Allied Health Professional. Experience in emergency and primary care settings and the ability to work with multidisciplinary teams are essential.
