Job description
Overview

Under the direction of their line manager, the post holder will undertake support planning, provide one-to-one, group support and interventions to improve the lives of unpaid Carers, families and friends affected by serious mental illness and or Substance misuse issues.

To equip unpaid Carers, families and friends with skills, knowledge, and coping strategies to ensure their own well-being is improved and maintained. This is to be achieved by visiting clients within their own homes, in the community, at hospital settings and or across groups established across, the North Denbighshire area, thus enabling the unpaid Carers to manage the impact of their caring role.

To work in partnership with other statutory, voluntary, private sector organisations and community facilities to ensure everyone can access other existing services.

Cyflog: £25,027.20 per annum (pro rata)
