Job description

As a Care Navigator, you'll be at the heart of our practice, ensuring that patients and visitors receive a warm welcome and assistance. This busy and diverse role requires excellent communication and interpersonal skills, as well as the ability to handle challenging situations with professionalism and calmness. You'll be adept at juggling multiple tasks and prioritising them effectively throughout the day.

You'll be part of our friendly and supportive Care Navigator team, working approximately 27 hours per week. Flexibility is key, as you'll occasionally cover for absences or annual leave, with availability required between 8am and 6pm.

Main Duties of the Job:

  • Be the first point of contact for patients and visitors, ensuring all enquiries are handled promptly and professionally.
  • Signpost patient requests via our Online Consultation portal and allocate appointments to the most appropriate service or clinician.
  • Support clinicians to ensure surgeries run smoothly and efficiently.
  • Undertake a range of administrative tasks to support the practice.
  • Work flexibly to cover co-worker absences as needed.
