  • Work in partnership to assess needs and create personalised Pathway Plans that reflect their goals, strengths, and vulnerabilities.
  • Support access to legal advice, health services, education, training, and employment opportunities.
  • Assist with securing safe accommodation and understanding financial entitlements.
  • Collaborate with carers, families, and partner agencies to identify needs and contribute to service development.
  • Travel across the country as required to meet care leavers in person, ensuring continuity of support and building strong, trusting relationships.
  • Strong communication and relationship‑building skills, including negotiation and mediation.
  • Ability to assess risks and needs, analyse information, and work independently.
  • Proficient IT skills and ability to produce clear, structured written reports.
  • Enjoy flexible working options, including a 9‑day fortnight and hybrid arrangements to support work‑life balance.
  • Access to Health Assured's comprehensive Employee Assistance Programme to support your physical and mental well‑being, including 24/7 telephone support, a suite of online resources, and legal and financial advice.

  • Strong communication and relationship‑building skills, including negotiation and mediation.
  • Ability to assess risks and needs, analyse information, and work independently.
  • Proficient IT skills and ability to produce clear, structured written reports.
  • Commitment to inclusive, non‑discriminatory practice.
