Overview

Calling all frailty-minded practitioners and generalists. Are you Advanced Clinical Practitioner or a training ACP interested in older people’s care, frailty or palliative care? Do you often feel you could do more to improve quality of care for older people?

Join the One Weston Care Home Hub in North Somerset, based at 168 Medical. This is an opportunity to work within a multi-disciplinary team dedicated to high-quality care for older people, with a focus on holistic medicine, collaboration, and continual learning.

This role may offer a hybrid arrangement with the primary care team at 168 Medical Group, blending generalist skills with specialist practice.

Primary responsibilities
  • Provide clinical assessment of care home residents with proactive and reactive medicine, in collaboration with the MDT.
  • Undertake holistic assessments and devise creative, patient-centred solutions; develop dynamic, personalised care plans including treatment escalation planning and ReSPECT forms where appropriate.
  • Ensure continuity of care for residents, families and care home staff; support complex decision making when clinical lead of the day is required.
  • Take clinical responsibility for decisions and ongoing management, drawing on MDT support as needed.
  • Contribute to peer learning and education through significant event analysis, case discussions and other formats; provide supervision and feedback to students and MDT members.
  • Promote adult safeguarding and advocate for high-quality, patient-centred care for older or vulnerable adults.
  • Record data and assessments promptly in patient records systems; ensure appropriate use of read codes and templates with awareness of QOF targets and local DES specifications.
  • Manage prescriptions in line with BNSSG formulary, in collaboration with the pharmacy hub.
  • Initiate and interpret invasive and non-invasive diagnostics, tailored to the patient’s degree of frailty and treatment escalation plans.
  • Contribute to continuous improvement and development of care pathways for older people; participate in QIP and audits; review medications via Structured Medication Reviews with pharmacists.
  • Lead or participate in specialist MDT meetings (dementia/mental health, palliative care, complex care) as part of the MDT and with community colleagues.
  • Provide outreach input to care home cases across Pier Health PCN as needed, supporting safeguarding processes and practice needs.
How we work

Daily routine includes a whole-team check-in (in person or via Microsoft Teams) to discuss wellbeing, problems, ideas and workload. The clinical lead of the day is assigned during this meeting.

Typical activities include ward rounds in care homes, comprehensive geriatric assessments, care planning, engaging with families and other partners, and managing acutely unwell residents. There is protected time for learning, teaching, supervision, quality improvement, and meetings, conducted via telephone, video, email or face-to-face. Regular 1:1 development meetings support career progression. We also receive regular MDT input from a geriatrician and mental health colleagues at AWP.

Where

Most work takes place at our town centre Care Home Hub in a newly built surgery and in surrounding care homes, with scope to work remotely at times.

Person specification

Experience (Essential):

  • At least 5 years’ experience working with older people in a community or emergency healthcare setting at Band 6 AfC equivalence or above
  • Substantial experience with complexity
  • Autonomous working within the 4 pillars of advanced practice

Experience (Desirable):

  • Primary Care, Community nursing and therapies, or emergency care practitioner background
  • Additional experience in: Palliative Care, Mental Health, Learning Disability, Dementia

Qualifications (Essential):

  • Registration with a health Governing Body (e.g., NMC, HCPC)
  • Nursing or AHP related degree
  • Postgraduate courses related to end-of-life care or recognising unwell patients
  • Independent/ Non-medical Prescriber V300 (or willingness to work towards)

Qualifications (Desirable):

  • Advanced Practice MSc
  • Diploma of Geriatric Medicine

Skills (Essential):

  • Experience working with Frailty and conducting holistic assessments
  • IT literate (MS Teams, EMIS, MS Office)
  • Good interpersonal skills, effective time management, planning, prioritisation and handover
