Care Coordinator – Supported Living (Adults with Learning Disabilities)

Location: Preston

Salary: £30-31k + on-call allowance

Contract: Full-time Monday - Friday

Are you passionate about making a real difference in the lives of adults with learning disabilities? Join our friendly and supportive charity team in Preston as a Care Coordinator.

What You’ll Do
  • Oversee a team of support workers in our supported living services, ensuring high-quality, person-centred care.
  • Manage rotas, liaise with families and professionals, and provide on-call support as needed.
  • Lead and organise the team to deliver safe, responsive, and empowering care.
Key Responsibilities
  • Coordinate day-to-day operations of supported living services
  • Supervise and support care staff, providing guidance and mentorship
  • Ensure care plans are up-to-date and person-centred
  • Maintain accurate records and monitor quality standards
  • Take on on-call duties to provide guidance and support out-of-hours
  • Liaise with families, healthcare professionals, and local authorities
