Summary

As a Care Coordinator at Elderflower Care Services, you will play a vital role in managing and coordinating the delivery of care services to our clients. This position is essential for ensuring that our clients receive the highest standard of care while maintaining their dignity and independence.

Responsibilities
  • Coordinate and oversee the delivery of personalized care plans for clients.
  • Communicate effectively with clients, families, and care staff to ensure quality service.
  • Monitor client progress and adjust care plans as necessary to meet changing needs.
  • Conduct regular assessments to evaluate the effectiveness of care provided.
  • Ensure compliance with health and safety regulations and company policies.
  • Support the recruitment and training of new care assistants as needed.
  • Take part in an On-Call rota with addition pay for this.
Requirements
  • Previous experience in a similar role within the home care or healthcare sector is preferred.
  • Strong organizational skills and attention to detail.
  • Excellent communication skills, both verbal and written.
  • Ability to work collaboratively with a diverse team of caregivers and clients.
  • Basic office skills including proficiency in computer applications.
