Overview

Type: Full-time. Driving Requirement: A valid driving licence and access to a car are essential for this role.

We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will ensure that clients receive the highest standard of care while smoothly navigating their healthcare journey. This role requires strong organisational skills, excellent communication, and the ability to work effectively in an office environment while supporting field-based staff.

Key Responsibilities
  • Coordinate and oversee care plans, ensuring that all required services are delivered efficiently and on time.
  • Serve as the primary point of contact for clients and their families, responding promptly to inquiries and concerns.
  • Maintain accurate and up-to-date records of client interactions and care plans within the office system.
  • Liaise with healthcare providers and other professionals to ensure comprehensive, person-centred care.
  • Schedule appointments and follow-up visits, monitoring client progress and adjusting care plans as needed.
  • Provide leadership and supervision to care staff, supporting high-quality service delivery.
  • Deliver care in line with best practices, policies, procedures, and relevant legislation, under the direction of the Manager.
  • Support administrative tasks such as filing, data entry, and documentation maintenance.
  • Keep skills current through ongoing training and professional development.
Experience & Skills
  • Previous experience in healthcare, social care, or a similar setting is highly desirable.
  • Strong organisational and time-management skills with keen attention to detail.
  • Excellent verbal and written communication skills for engaging with clients, families, and professionals.
  • Proficiency with office software and care management systems.
  • Compassionate and empathetic approach to client care.
