A social care service provider in Sunderland is seeking a Care Co-ordinator to deliver exceptional person-centred care for individuals with disabilities. We are seeking to recruit a Care Co-ordinator (Level 1) into our Learning Disabilities Team who can deliver exceptional standards of person‑centred care and support to those with a high level of support needs as well as those who may be at risk of displaying behaviour that challenges. We are keen to recruit a candidate who is able to support and enable our customer to lead and maintain a fulfilling life whilst ensuring that their daily individual care and support needs are met whilst ensuring their quality of life is paramount. These needs should be met with integrity by those who can help us ensure our company values are at the heart of everything we do.
