Job description

We are seeking to recruit a Care Co-ordinator (Level 1) into our Learning Disabilities Team who can deliver exceptional standards of person‑centred care and support to those with a high level of support needs as well as those who may be at risk of displaying behaviour that challenges. We are keen to recruit a candidate who is able to support and enable our customer to lead and maintain a fulfilling life whilst ensuring that their daily individual care and support needs are met whilst ensuring their quality of life is paramount.   • Helping with preparation of meals and drinks.
  • Support with domestic household tasks.
  • Administer and record medication.
  • Support with money management.
  • Encouraging community trips and facilitating customers participation.
  • Support with household and personal shopping.
  • Support with meeting friends and family.
  • Support with high standards of nutrition.
  • Adhere to health and safety legislation.
  • Cleanliness and intimate personal hygiene.
  • Opportunities for employment or education.
  • Provide emotional support.
  • Active support techniques and positive behaviour support techniques to increase skill level and achieve set outcomes (training will be provided).
  • Support customer to plan holidays and support them if needed (additional payment).
