About the Organisation

Role Overview

Cardiology ANPs at Glan Clwyd Hospital provide a high‑quality, comprehensive and innovative nurse‑led service for patients with Acute Coronary Syndromes. They deliver timely care and interventions for Acute ST‑Elevation Myocardial Infarction (STEMI) and high‑risk Non‑ST‑Elevation Myocardial Infarction (NSTEMI) patients across North Wales and complete the care continuum with our "Post‑Intervention Clinics". The team comprises six ANPs (F/T and P/T) who work closely with all members of the cardiology multi‑disciplinary team to deliver inpatient and outpatient care, advise clinicians and provide education and training to primary and secondary care staff.

Responsibilities
  • To work alongside the ANPs co‑ordinating Primary PCI calls.
  • Identifying patients with acute coronary syndrome within the emergency quarter.
  • Assist and advise allied health professionals MDT on ECGs and treatment of STEMI and ACS.
  • Working closely with CCU and MDT to ensure cardiac patients receive quality care in the correct environment.
  • Complete local and national audits.
  • Teach junior members of staff and other members of MDT.
  • Learn to identify critically ill cardiac patients within the acute‑setting including those who require PPCI.
  • Learn to escort the patient throughout their PPCI pathway acting as support to the Catheter Laboratory Team.
  • Work alongside DSMP & Cardiology ANP to develop confidence and competence to autonomously run RACPC with responsibility for clinical standards, competence assessment and supervision of other health professionals.
  • Work alongside DSMP & Cardiology ANP to gain expert clinical knowledge to be able to autonomously advise in the management of ACS patients in all areas of the hospital including Unscheduled Care.
  • Work collaboratively with primary care colleagues to understand the specialist cardiology teams that work across primary and secondary care.
  • Develop a knowledge base that enables delivery of specialist education and training to a broad range of health professionals including nurses, doctors and ambulance personnel.
  • To undertake research and lead clinical audit in own speciality with organisational responsibility for compliance with relevant national audit programmes.
  • Complete clinical competencies to support the Cardiac Catheter Laboratory in the North Wales Primary Percutaneous Cardiac Intervention Service (PPCI).
  • To learn to recognise patients with changing health status and to take appropriate actions.
  • Communicate highly complex and highly sensitive information with patients, families and the healthcare team.
  • Responsible for the safe use of highly complex patient monitoring and support equipment within the ED/CCU/Catheter Lab.
  • To learn interpretation of laboratory and radiological results and initiate appropriate patient management.
  • Maintain own clinical development by keeping abreast of new treatments and technologies within cardiology.
  • To rigorously review with a supervisor all aspects of the patient's plan of care and identify and address issues that may result in a sub‑standard service.
  • Work alongside Cardiology ANP to develop the ability to safely triage referral‑received patients with suspected Acute Coronary Syndrome (ACS) and conduct a comprehensive clinical assessment including examination of their healthcare needs.
  • If appropriate, transfer patients for primary PCI to the Catheter Lab and to CCU/ward if necessary.
  • Evaluate the care given to patients, order necessary investigations, act upon results, provide treatment and care both individually and through referral to Cardiologists or specialists as appropriate.
  • Provide expert professional advice and liaise with patients, the Emergency Department, Acute Admissions Unit, Cardiac Catheter Laboratory, Coronary Care Unit and Cardiology Ward colleagues.
