A healthcare staffing agency is seeking a Band 8a Pharmacist for a locum position in the Community Frailty Team in Plymouth. Qualifications

  • Knowledge of polypharmacy and deprescribing advantageous.
  • HCPC registration necessary.
  • Full driving licence required for travelling between sites.

Responsibilities

  • Clinically screen medication charts on the ward.
  • Screen discharge prescriptions and one-stop orders.
  • Complete medicines reconciliation.
  • Handle ward MI queries and work in MDT settings.
  • Conduct ward CD audits and order medicines.
  • Collaborate closely with Medicines Optimisation technicians.

Locum Band 8a Pharmacist in the Community Frailty Team – Plymouth, UK

Are you ready to embark on an extraordinary journey as a Band 8a Pharmacist within our Community Frailty Team in Plymouth? This ongoing locum role offers £36 per hour. Join us on a full-time basis and make a real difference in the lives of those in your community.

What you will do
  • Clinically screen medication charts on the ward
