Band 8a Neuro navigator for Barnet and OT/PT lead

Are you an experienced Allied Health Professional looking for a new challenge in neurorehabilitation?

Join our team at the Royal Free Neurological Rehabilitation Centre (NRC) and help shape patient care pathways across acute, inpatient, and community settings.

Based within the Community Neurological Conditions Management team (CNCMT) at Edgware Hospital, you will manage an active caseload, work closely with acute hospitals, specialist neuro rehab services, and community teams, and collaborate with North Central London neuro navigators. As part of the NRC senior leadership team, you will deputise for the community team lead and support the strategic planning of the NRC and be the lead OT/ PT in the service .There are opportunities for service development, clinical leadership development , and access to CPD and formal training programmes.

You will take a lead in developing provision of specialist case management for complex needs within the community team

You will be the lead for OT/PT which includes expert clinical support to the wider service and supervision of IP and community based B7 OT's / PT's

Experienced Occupational therapist / PhysiotherapistBackground in neurorehabilitation and complex patient management with post acute or community settings Strong communication, partnership skills and leadership skills

Main duties of the job

The post holder will lead the management of Barnet patients with complex neuro-rehabilitation needs throughout the neurological rehabilitation pathway from acute care, specialist inpatient neurorehabilitation and into the community by carrying an active caseload and by implementing and review of a collaborative pathway

  • Provide expert assessment as an autonomous practitioner of patients being recommended for treatment to specialist rehabilitation units and to ensure that patients with specialist neuro rehabilitation needs are referred to the most appropriate service to meet their needs
  • Monitor patients in terms of their rehabilitation needs who are in specialist rehabilitation settings in liaison with North Central London ICB
  • Work in partnership with other health and social care providers, promoting an integrated approach to the management of complex neurological conditions in Enfield
  • Provide a case management approach for complex cases in the CNCMT and other Barnet patients, acting as a central point of contact for the multidisciplinary team including the patient and their family.
  • Be lead OT / PT in the service to provide expert Clinical support and decision making

Education & professional Qualifications
  • Qualification in an Allied Health Profession (OT/PT) and HCPC registered

  • Specialist Section in Neurosciences e.g. SSNP
  • Masters level qualification in an Allied Health Profession or equivalent experience
  • Member of relevant professional Body e.g. BAOT, CSP

Experience
  • Substantial experience within relevant clinical area e.g. Acute Neurosciences or Neurological Rehabilitation settings.
  • Experience of working as an Advanced Therapist (band 7) within rehabilitation
  • Experience of teaching and supervising less experienced staff or students.
  • Previous experience of leading change in a clinical environmentDetailed familiarity with discharge processes from secondary care
  • Experience of working effectively and efficiently as part of an MDT.
  • Substantial post-graduate work experience as a therapist (occupational therapist / physiotherapy)
  • Demonstrates ability to work autonomously and use own initiative
