A leading healthcare organization in Birmingham seeks a clinical lead for the Single Point of Access (SPA) service. You will provide clinical triage and expertise, work with multidisciplinary teams, and contribute to reducing hospital admissions. An exciting opportunity has arisen to be part of a brand-new Single Point of Access (SPA) service designed to transform how we deliver care and support to our patients across urgent care and community services. This is your chance to shape a pioneering, person-centred service right from the start!
The SPA will act as the central hub for coordinating clinical referrals, triaging urgent needs, providing advice and directing patients to the right services at the right time. Whether you're answering a call, reviewing referrals, or coordinating rapid support, you will play a key role in ensuring patients and professionals receive timely, efficient, and compassionate responses. Work alongside WMAS and other system partners to deliver joined-up care.
To provide advanced clinical expertise and leadership within the Single Point of Access to reduce unnecessary hospital admissions and contribute to effective admission avoidance and facilitate early hospital discharge. This includes effective history taking, clinical triage, diagnosis, and the implementation of a range of care services for patients with acute undifferentiated, undiagnosed conditions and long-term conditions.
The post holder will demonstrate critical thinking in the clinical decision-making process; identify gaps within the service and assist with service improvement and development in line with national and local strategies.
The post holder will be responsible for facilitating effective teamwork and provide leadership, mentorship, teaching and support within the Adult Community Services Division.
The post holder will be expected to rotate into clinical teams every 3-4 months to remain up to date with the changing landscape of community services and own practice.
The post holder will be expected to work within a flexible working pattern over a 7 day week.
