Job Overview

Are you an experienced CHC professional ready to take a strategic step into a role where your expertise directly shapes care for our most vulnerable residents? Mid and South Essex ICB is seeking a dedicated Band 7 Nurse Advanced to join our dynamic All Age Continuing Care service.

Responsibilities

This is more than a management role; its a chance to become a pivotal leader. You will autonomously manage a complex caseload, lead Multi‑Disciplinary Teams through critical eligibility decisions, and commission bespoke care packages that truly change lives. Your deep understanding of the National Framework will be essential.

As a Band 7 Advanced Practitioner, you will be the lead expert for a complex caseload within our All Age Continuing Care service. Your core duties will involve chairing Multi‑Disciplinary Team meetings to assess eligibility using the Decision Support Tool, and designing, commissioning, and reviewing personalised care packages. You will provide professional supervision and guidance to junior team members, ensuring the highest standards of practice.

A key aspect of the role involves safeguarding our most vulnerable patients, which includes applying your knowledge of the Mental Capacity Act and Deprivation of Liberty Safeguards. You will act as a vital link between patients, families, and a wide network of health and social care providers across mid and south Essex to ensure seamless, person‑centred care.

Qualifications

We are looking for a confident leader with proven CHC experience. If you also possess knowledge of the Court of Protection and Deprivation of Liberty Safeguards (CoP DoLS), you are the ideal candidate to navigate our most sensitive cases.
