  • Provide expert health advice and support to children and young people living within the Home.
  • Carry out comprehensive health assessments, known as CHATs, and develop personal health plans for the residents.
  • Deliver targeted health promotion and training sessions for staff, children, and their families.
  • Collaborate with other health and social care professionals to ensure young people have access to universal and specialist health services.
  • Work proactively to improve health outcomes through personalised packages of care, supporting young people in their transition back to the community.
  • Promote a strength‑based and trauma‑informed approach to care in line with the ethos of the Health and Wellbeing Team.
  • Contribute to the development of an inclusive, innovative service shaped by listening to the needs and aspirations of young people.
