A vacancy has arisen for a Band 5 community mental health lead depot nurse (Key Worker) to join the later life Complex Intervention and Treatment Team (CITT) based in the Victoria Centre, Swindon.
Applicants will need to demonstrate knowledge in the areas of the delivery of mental health care in the community, with particular reference to the needs of Older People, together with an understanding of both the PWP and Care Management processes.
You will be required to provide key interventions for an allocated caseload of service users with substantial and complex mental health needs. Potential additional interventions could involve being responsible for the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as key worker for a defined group of service users. Support will also be offered to family and carers.
Applicants will be expected to demonstrate a high level of decision making skills and to work independently. Applicants will also need to be proficient with the use of IT systems for both data and documentation purposes.
