The post holder will work as part of a designated team delivering evidence-based high quality clinical care that focuses on the direct needs of the patient, including those with chronic conditions. The post holder will work under the direction and support of a Registered Nurse by following the Health Board\'s protocols, policies, procedures, pathways, clinical governance requirements and care plans.

The role requires effective communication and practice in accordance with the standards and statutory requirements and operating within the boundaries of the role and essential competencies.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
You must hold the QCF Level 4 Qualification - Foundation Degree in Health Care Level 4 or be working towards it.
The post holder should be able to speak Welsh where possible; Welsh language skills are desirable.
