• Assess needs and implement care plans for individuals.
• Provide support across Acute ward, A&E, and community settings.
• Maintain accurate records and produce reports for various audiences.
• Work flexibly and assist customers, carers, and families.
• Collaborate with integrated discharge teams for safe patient transitions.

Skills

Ability to analyse and interpret information

Ability to work independently and as part of a team

Ability to work in a multi-agency environment

Sound and accurate IT and keyboard skills

Education

GCSE English and Maths or equivalent

NVQ Level 3 in Care

Experience in a Social Care environment
