A local charity organization in the United Kingdom is seeking an Ageing Well Coordinator. This role requires a people-focused individual experienced in support work within health and social care. 📍Triangle Primary Care Network- NN14, NN18 X 15hrs
📍Grand Union Primary Care Network- NN1-NN5 X 22.5hrs

As an Ageing Well Coordinator, you’ll be a key member of a multidisciplinary team, working closely with GP surgeries and community partners within PCN. Your role will involve:

You may already be working as a primary care practitioner, within PCN primary care network or support worker.
