Main duties of the job

Job Summary

As an Advanced Nurse Practitioner (ANP)at New Horizons Medical Partnership you will provide an efficient on the dayservice to our patients, working across our current two GP Surgery sites,Forest Gate Surgery and Totton Health Centre.

Working independently, face to face orvia telephone consultation, you will be accountable for delivering safe,clinical decision making and expert care to a high standard.

If you have an interest in education,you could take part in a new to the practice initiative by being involved in thesupervision of student nurses. New Horizons is a GP Registrar trainingpractice, and we are interested in expanding this to include student nurses.

About us

Within the ANP team, there is aclinical lead ANP and a partner lead. We provide supervision sessions, teachingat a PCN level and monthly clinical team meetings. There will always be asenior doctor for support, and we practice within safe capacity limits. Werecognise the importance of time to decompress and there is a timetabled teamcoffee break part way through every session. Outside of work there are eventsorganised too.

Job responsibilities

Acting within your professionalboundaries your duties will include all or part of the patient journeyincluding initial history taking; clinical assessment; diagnosis; treatment;prescribing and evaluation of their care.

You will also contribute to theleadership and mentorship of the treatment room team.

Formulate a management plan for thepatient, using other sources of healthcare professionals (such as GPs orPharmacists), if required, to support diagnosis or next course of action.

Prescribe medication as requiredwithin your clinical competence.

Your responsibilities could alsoinclude but not limited to other aspects of Nursing care like Chronic diseaseclinics and learning disability clinics. We would welcome any additionalroles/duties that you might like to pursue outside of the on the day service.

You will be professionally and legallyresponsible and accountable for all aspects of clinical decision making withinyour clinical competence.

You will needto cover PAN PCN working as and when required.

Training:

Post-holders will be encouraged andsupported to undertake any training required to ensure appropriate roledevelopment.

Personal/ProfessionalDevelopment:

Thepost-holder will participate in any training programme implemented by thePractice as part of this employment, such training to include:

Participation in an annual individual performancereview, including taking responsibility for maintaining a record of ownpersonal and/or professional development.

Takingresponsibility for own development, learning and performance and demonstratingskills and activities to others who are undertaking similar work.

Person Specification

Qualifications

• You will have either
• Graduated from a Centre for advancing Practice accredited MSc advanced practice programme or completed the centres ePortfolio (Supported) route; or
• Enrolled as a trainee on a Centre for Advancing Practice accredited MSc advanced practice programme; or
• Enrolled on and progressed past the initial Learning Needs Analysis (LNA) stage of the Centre for Advancing Practice.
