We're looking for an experiencedAdvanced Nurse Practitioner to join our Neighbourhood Access Service, part ofthe innovative New Models Programme. You'll work autonomously and as part of amultidisciplinary team within primary care to provide expert assessment,diagnosis, treatment, and evaluation for patients presenting with minorillness, acute conditions, and common general practice presentations, includingwomen's health.

As a Nurse Practitioner you willprovide autonomous care for our patients presenting with minor/acute illness,and common general practice presentations including those relating to women'shealth. You will demonstrate safe clinical decision making and provide expertcare by undertaking initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.
Working with GPs and nursingteam in the Practice you will continually review and evaluate services and workwith the management team in implementation of change.

The Northamptonshire Rural East& South (NRES) Neighbourhood, consisting of 7 GP Practices serving 88,938patients, is developing an innovative new model of care as part of the NHS NewModels Programme.
You will be working with a teamof GPs and specialist ANPs to deliver same day care to the Rural East &South population across our three hub sites.

Clinical
Provide clinical care to our patients. This will includereducing the demand on GP time by the assessment and management of patients bymeans of telephone triage and clinic appointments.
Use advanced skills and expert knowledge to assess thephysical and psycho social needs of patients with common primary careconditions and minor illness, instigating and providing therapeutic treatmentsbased on best available evidence to improve health outcomes.
Use advanced skills and expert knowledge to makecomprehensive and focussed assessments, independently and proactively managingcare of the patients.
Promote patient independence and self-care whereappropriate.
Undertake the role of independent non-medical prescriberin the treatment and care of patients
Review pathology results to ensure appropriate treatmentand care
Support and lead the practice for the delivery of theQuality and Outcomes Framework and NHS England/ICB/Federation Contracts.
Regulatory
Respond to complaints and clinical incidents/significantevents as appropriate to ensure robust systems are in place for improvingpractice through joint learning.
Undertake at least one clinical audit a year. Evaluatedata and present findings to the Practice Clinical team. Implement changeswhere appropriate.
Adhere to the Practice Policies and Procedures includingInfection Control, Health and Safety

Personal /Professional Development
Maintain own personal/professional development plan andportfolio thereby meeting the requirements for revalidation through appraisalof own performance.
Maintain NMC registration and professional development inline with expectations of the role
Work at all times within the NMC Code of ProfessionalConduct.
Provide Mentorship and training of new recruits orstudents as and when required.
Participate in the Practice Performance Review process.
