Job summary

A local healthcare provider in Northampton is seeking an experienced Advanced Nurse Practitioner to deliver high-quality, same-day care within a multi-disciplinary team. The ideal candidate will work autonomously, providing expert assessment, diagnosis, and treatment for patients with minor illnesses and common conditions, including women's health. This role offers the opportunity to shape integrated services for a large patient community and participate in professional development across member practices.

Qualifications

  • Must provide autonomous care for patients with minor/acute illness.
  • Experience in implementing therapeutic treatments based on best evidence.
  • Ability to review pathology results for appropriate treatment.

Responsibilities

Work within our practices in theNorthamptonshire Rural East & South (NRES) Neighbourhood to deliverhigh-quality, same-day care to our patients.

We're looking for an experiencedAdvanced Nurse Practitioner to join our Neighbourhood Access Service, part ofthe innovative New Models Programme. You'll work autonomously and as part of amultidisciplinary team within primary care to provide expert assessment,diagnosis, treatment, and evaluation for patients presenting with minorillness, acute conditions, and common general practice presentations, includingwomen's health.

Main duties of the job

As a Nurse Practitioner you willprovide autonomous care for our patients presenting with minor/acute illness,and common general practice presentations including those relating to women'shealth. You will demonstrate safe clinical decision making and provide expertcare by undertaking initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.

Working with GPs and nursingteam in the Practice you will continually review and evaluate services and workwith the management team in implementation of change.

About us

The Northamptonshire Rural East& South (NRES) Neighbourhood, consisting of 7 GP Practices serving 88,938patients, is developing an innovative new model of care as part of the NHS NewModels Programme.

You will be working with a teamof GPs and specialist ANPs to deliver same day care to the Rural East &South population across our three hub sites.

Job responsibilities

Clinical

Provide clinical care to our patients. This will includereducing the demand on GP time by the assessment and management of patients bymeans of telephone triage and clinic appointments.

Use advanced skills and expert knowledge to assess thephysical and psycho social needs of patients with common primary careconditions and minor illness, instigating and providing therapeutic treatmentsbased on best available evidence to improve health outcomes.

Use advanced skills and expert knowledge to makecomprehensive and focussed assessments, independently and proactively managingcare of the patients.

Promote patient independence and self-care whereappropriate.

Undertake the role of independent non-medical prescriberin the treatment and care of patients

Review pathology results to ensure appropriate treatmentand care

Support and lead the practice for the delivery of theQuality and Outcomes Framework and NHS England/ICB/Federation Contracts.

Regulatory

Respond to complaints and clinical incidents/significantevents as appropriate to ensure robust systems are in place for improvingpractice through joint learning.

Undertake at least one clinical audit a year. Evaluatedata and present findings to the Practice Clinical team. Implement changeswhere appropriate.

Adhere to the Practice Policies and Procedures includingInfection Control, Health and Safety

Provide Mentorship and training of new recruits orstudents as and when required.

Participate in the Practice Performance Review process.

Person Specification
Experience
  • In depth knowledge base in the management of patients with chronic illness and minor illness
  • Ability to maintain and monitor high standards of evidence based care
  • Ability to direct and co-ordinate care working autonomously and collaboratively
  • Understanding of clinical governance issues
  • Evidence of improving the patient's experience
  • Evidence of leading a service project at practice level
  • Undertaking research or involvement in clinical research programmes
  • Experience in writing clinical policies and protocols
  • Counselling experience
Qualifications
  • RGN with a minimum of 5 years post registration experience
  • Independent non-medical prescriber
  • Community/practice nurse qualification or equivalent experience
  • Extensive clinical experience in primary nursing
  • Evidence of continuing professional development
  • Post registration qualification in minor illness/injury, women's health/mental health/paediatrics
  • Teaching qualification
  • Recent attendance on a leadership skills development programme
