A healthcare organization in Molesey is seeking an experienced Advanced Nurse Practitioner to enhance patient care. The role involves providing advanced care, managing chronic diseases, and ensuring patient safety. Candidates must be registered as Independent Nurse Prescribers and have experience in primary care. The AdvancedNurse Practitioner (ANP) has been assessed as competent in practice using theirexpert clinical knowledge and skills. They have the freedom and authority toact and make autonomous decisions in the assessment, diagnosis and treatment ofpatients.
The ANPprovides an opportunity for patients to avoid the need to be referred to a GP,thereby enabling them to receive timely care and negate unnecessary delay inreceiving treatment.
ANPs inprimary care can develop close, long-term relationships with their patients andwork in partnership with them to achieve optimum health. They are autonomous inmaking decisions based on assessment, diagnosis and interpretation of testresults. They can independently prescribe appropriate medication, evaluate or referto other specialists if necessary.

Implementhealth promotion and wellbeing programmes
Managepatients presenting with acute or chronic medical conditions
Evaluateindividual specialised treatment plans for chronic disease patients
Identify andmanage patients at risk of developing long- term conditions
Provideadvanced care to patients as required in accordance with clinical basedevidence
Request,process and interpret pathology services and other tests as necessary
Deliverpatient care as necessary, referring patients to secondary/specialist care asrequired
Maintainaccurate clinical records in conjunction with legislation
Ensure codingis used effectively
Maintainchronic disease register
Prioritisehealth issues and intervene appropriately
Support theteam with clinical emergencies
Recognise,assess, refer patients presenting with mental health needs
Be anextended and supplementary prescriber, adhering to extant guidance
Supportpatients in the use of their prescribed medicines (within own scope ofpractice), reviewing annually as required
Contribute topractice targets (QOF, etc.), complying with local and regional guidance
Liaise withexternal services/agencies to ensure the patient is supported appropriately
Delegateclinical responsibilities appropriately, ensuring safe practice (within thescope of practice)
Support theclinical team with all safeguarding matters in accordance with policy
Deliver opportunistichealth promotion where appropriate
Looking for an Advanced Nurse Practitioner to start as soon as possible with 4-6 sessions a week, we are located inleafy Surrey, being a medium-sized GP practice with a patient list of 8,500+.Our dedicated team includes:
