A Community Medical Centre in England is looking for an Advanced Nurse/Clinical Practitioner specializing in frailty. In this part-time role, you will lead the care for older patients in the community, conducting assessments and managing chronic conditions. You will work alongside a dedicated clinical team and be actively involved in multidisciplinary meetings. To act as line manager for the Frailty Nurse Practitioner and to take the lead in providing and maintaining a flexible clinical service for our older demographic in the community with an emphasis on care delivered at home.
This is a part time role working 27 hours per week.

You will be fully supported by and work alongside a fulltime Nurse Practitioner and assigned Visiting GP along with the broad clinical team at the practice comprising of GPs, Clinical Pharmacists, Paramedics, PracticeNurses, Advanced Clinical Practitioner and Health Care Assistants. It is an permanent role. Full induction and training is given. We offer a generous NHS pension scheme, staff uniform, five weeks annual leave and bank holidays prorata.
