Qualifications

Advanced Clinical Practitioner

Advanced Clinical Practitioner SDEC Band 8a - 37.5 hours per week

Join our dynamic team as an Advanced Clinical Practitioner (ACP) in the rapidly expanding Same Day Emergency Care (SDEC) unit. You will work closely with a dedicated workforce of ACPs, alongside the ACP Lead and Clinical Lead for SDEC, ensuring high-quality patient care. As part of an innovative service shaping the future of urgent care within the NHS, we are committed to professional growth and excellence.

The post holder is an qualified Advanced Care Practitioner, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision making and expert care for patients within the Same Day Emergency Care unit with the Urgent Care provision.

Main duties of the job
  1. To act as a role model of expert clinical practice.
  2. To ensure that documentation is maintained to a high standard. Be a competent user of and participate in the ongoing development of the electronic patient record.
  3. Ensure that patients receive high quality clinical care including medical diagnosis.
  4. Ensure that effective discharge planning and health promotion is carried out.
  5. To monitor the quality and continuity of patient care and ensure standards are met.
  6. Establish and maintain effective communication with nursing, medical and paramedical colleagues to provide optimum patient care.
  7. Communicate effectively with patients, relatives, and colleagues.
  8. Participate in audit, taking action to address improvements required
  9. Practice in such a way that inspires confidence in all people using the service
  10. Provide advanced levels of practice, knowledge and skills within the Urgent Care Pathway.
  11. Develop and implement care pathways for the management of groups of patients within Urgent Care.
  12. Promote evidence based practice through the use of the latest research based guidelines and the development of practice based research.
  13. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
