Overview

Are you an experienced Advanced Nurse or Clinical Practitioner passionate about delivering high-quality patient care? Practice Plus Group is seeking dedicated healthcare professionals to join our Out of Hours team and make a real difference in the community. We are a leading provider of urgent care services across the UK, committed to delivering safe, effective, and compassionate care to patients when they need it most outside of regular GP hours.

In OOH we assess and treat patients who require urgent unscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed. Our referrals come from either 111, via a dedicated health care professional line used by paramedics and district nursing teams or from the AED or MIU. We are looking to expand our team of Face to Face clinicians in bases across George Eliot Hospital Nuneaton, Warwick General Hospital, Rugby St Cross Hospital, or Coventry Walk In Centre, as well as via home visits where appropriate.

Main duties of the job

• Deliver comprehensive clinical assessment, diagnosis, and treatment for patients presenting with urgent healthcare needs.
• Conduct consultations from the clinical base, providing face-to-face care in a fully equipped environment.
• Undertake home visits to patients requiring assessment or treatment in their own residence, offering a personalized and compassionate approach.
• Manage telephone triage, providing expert clinical advice, risk assessment, and directing patients to appropriate care pathways.
• Work autonomously while collaborating closely with a multidisciplinary team to ensure seamless, patient-centred care.
• Maintain accurate clinical records and adhere to Practice Plus Group policies and NHS guidelines to uphold high standards of care.

About us

• Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification
