What you will do
  • Facilitate and support individuals to complete a needs assessment using a strengths-based approach, considering support networks within families and the wider community.
  • Consider approaches and early interventions to delay or prevent the development of needs in individuals, leveraging universal and preventative services.
  • Develop independence plans using a person-centred approach in collaboration with both statutory and non-statutory agencies.
  • Consult and collaborate with other professionals to identify and meet the needs of service users and carers, fostering effective joint working practices.
  • Undertake safeguarding enquiries, establish protection plans, and perform safeguarding reviews, ensuring accurate and timely recording.
  • Assess and manage risk for individuals, families, carers, and communities while applying relevant policies and procedures.
  • Undertake statutory duties as required by legislation including the Care Act 2014, Mental Health Act, and Mental Capacity Act.
  • Promote multi-disciplinary partnerships and sometimes act as a liaison between the council and health and social care partners.
  • Maintain a positive and professional approach that enhances the professionalism, quality of service, and image of the council.
Requirements
  • Degree or equivalent in social work
  • Current Social Work England registration
  • Substantial frontline experience in UK Social Services as a qualified Social Worker
