Job description
Overview

Adults Social Worker - Community East Team (part-time 30 hours). Based with the East Cambridgeshire Team, Princess of Wales Hospital, Ely. The role focuses on delivering strengths-based, evidence-informed social work with adults who have care and support needs, within the Learning Disability Partnership South Team context.

Job responsibilities
  • Provide Care Act assessments, Mental Health assessments, safeguarding adults enquiries, and ongoing reviews in a timely manner for adults, including older people.
  • Manage caseloads, maintain accurate record-keeping, and work as part of a multidisciplinary team with strong organisational skills.
  • Work under tight timescales and commission cost-effective, personalised support to meet assessed needs as part of the reviewing process.
  • Contribute to safeguarding as part of Making Safeguarding Personal; development opportunities may be available.
  • Provide structured, positive supervision to social workers and CCO; support in complex case work; promote good practice in risk enablement, safeguarding, the Mental Health Act, Mental Capacity Act, and Court of Protection work.
  • Collaborate with partner agencies to support adults and carers; empower service users to maintain independence and choices, whether in the community or in care settings.
  • Explore opportunities for professional development (e.g., pursuing qualifications such as Practice Educator or Best Interest Assessor).
About you
  • Experience working with individuals with complex care and support needs; solid understanding of the Care Act 2014, Mental Capacity Act, and related legislation.
  • IT literacy; ability to promote independence, choice, and control; ability to work across locations and travel countywide.
  • Qualified Social Worker, registered with Social Work England; full driving license or ability to travel to multiple locations.
  • Flexible, proactive, and committed to social justice and NHS Continuing Healthcare principles.
