Job description

We are looking for a passionate and motivated Care Coordinator to join our Connecting Care Team East.

Our team supports adults aged 18 and over who have care and support needs, working to promote independence, dignity, and wellbeing. Connecting Care Team East is a highly supportive and friendly team with a wealth of experience in adult social care.

Responsibilities
  • Undertake conversation-based social care assessments, re-assessments, and reviews.
  • Develop outcome-focused support plans that reflect individual needs.
  • Take a proactive approach in responding to individual circumstances, signposting to appropriate services, and determining eligibility through to completion of assessments in line with the Care Act 2014.
  • Root work in a person-centred, strengths-based, and outcome-focused approach, ensuring that people remain at the heart of everything we do.
Qualifications
  • Knowledge of care management and an understanding of the Care Act 2014.
  • Well organised and up-to-date knowledge of relevant legislation, including the Mental Capacity Act 2005 and the Human Rights Act 1998.
  • Social work qualification is not essential.
