Provides nutritional services for the facility’s in-center and home patients as applicable to maximize nutritional status and improve clinical outcomes. Functions as a member of the interdisciplinary healthcare team, supporting the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities. Actively participates in process improvement activities that help patients achieve their individualized goals as determined by their physician.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
- Assess patients' knowledge of diet and kidney disease; provide education tailored to their learning needs regarding diet, kidney disease, and treatment impacts.
- Deliver detailed, patient-specific education to patients, families, and caregivers about nutritional status, lab results, fluid management, and disease-related parameters.
- Complete nutritional assessments and participate in comprehensive interdisciplinary assessments.
- Develop individualized care plans in collaboration with the interdisciplinary team, compliant with federal and state guidelines.
- Calculate diet prescriptions according to standards of practice.
- Document progress notes for all dialysis patients per FMCNA guidelines, maintaining accurate medical records.
- Offer ongoing nutrition assessments and counseling to help patients achieve and maintain effective nutritional status.
- Identify malnourished or at-risk patients; work with the team to implement interventions, review lab results, and adjust plans accordingly.
- Monitor adherence to nutrition therapy, document responses, and modify plans as needed.
- Collaborate on Urea Kinetic Modeling and other management strategies for mineral metabolism, anemia, and more.
- Follow FMCNA policies and participate in QAI and staff meetings.
- Coordinate with team members for Pre-ESRD education and other patient education initiatives.
- Review and interpret nutrition-related labs; counsel patients and recommend treatment adjustments.
- Communicate effectively with physicians and facility staff regarding lab results and treatment plans.
- Participate in staff meetings, quality improvement activities, and other relevant forums.
- Consult with the Corporate Dietitian as needed and maintain professional development and licensure.
- Ensure safety and cleanliness in work areas, perform other duties as assigned.
PHYSICAL DEMANDS AND WORKING CONDITIONS :
- Work environment typical of healthcare settings, with temperature control and moderate noise. May involve exposure to infectious diseases, desk work, patient interaction, and travel between facilities.

EDUCATION AND CREDENTIALS :
- Registered Dietitian credential as per the Commission on Dietetic Registration.
- Board Certified Specialist in Renal Nutrition encouraged.
- Current state licensure if applicable.
EXPERIENCE AND SKILLS :
- Minimum 1-year professional experience as a Registered Dietitian, preferably with renal care background.
- Leadership, organizational, communication, and teamwork skills; ability to work independently; basic computer skills.
