SALARY: Based on education and experience
TERM: ASAP – March 31, 2025 (with possibility of renewal)
Reporting to the Tribal Nursing Officer, the Tribal Diabetes Coordinator’s main focus is to oversee the coordination activities and delivery of Health Canada – Aboriginal Diabetes Initiative. The position will be responsible for the overall day-to-day operations, planning, development and implementation strategies for this program while actively supporting and collaborating with programs in each of the affiliated and independent communities in the IRTC region, programs & staff and other specialized service providers to assure the successful achievement of IRTC’s vision, mission and program objectives.
The Tribal Diabetes Coordinator will become a part of IRTC’s Health Department team and provide the leadership and health expertise in Diabetes Services to improve the health status of individuals, families and communities through actions aimed at reducing prevalence and incidence of diabetes, and its risk factors. Ongoing networking and communication with IRTC’s Health Director, Aboriginal Diabetes Initiative (ADI) Workers and Tribal Nursing Officer will ensure coordination and cohesiveness of services.
This position is responsible for promoting healthy life choices using a community development and population health framework. This work plan objective is to offer a broad range of advisory/consultative services and community-based capacity building that enhances the health and well-being of IRTC’s community members while focusing on health promotion, diabetes awareness, prevention and management.
